Our History
Founded in 1999 in the heart of Carmel, New York, Advanced Health NY & Carmel Medical Care, PC was born from a passion to deliver premier physical medicine services to our community. What began as a modest 600-square-foot space operating on a part-time basis quickly proved too small to contain our growing impact—we outgrew it in under a year.
By 2000, we expanded to a 1,000-square-foot facility, only to surpass its capacity within another year as our patient base flourished. From 2007 to 2010, our commitment to accessible care led us to open several additional locations across the region, broadening our reach while maintaining the highest standards of excellence.
In 2010, we settled into our current flagship home: a state-of-the-art 5,000-square-foot facility designed with innovation and patient comfort at its core. Then, in 2016, we made a strategic pivot to consolidate our vision. Pouring our energy into renovating, expanding, and introducing cutting-edge services right here in Carmel, we transformed this space into a comprehensive wellness hub—ensuring every visit reflects our enduring dedication to transformative health outcomes.

Emergencies: If you're facing a medical emergency and can't reach us, head straight to your nearest Emergency Room or Urgent Care facility for immediate attention.

Insurance: We proudly accept most major insurance plans for a wide range of our services. Contact our office to confirm coverage details.

Payments: Co-pays, co-insurances, and fees for uncovered services are due at the time of your visit to keep things seamless.

Returned Checks: A $30 fee may apply for any returned checks to cover administrative costs.

Appointments: Kindly provide at least 24 hours' notice for cancellations to avoid a fee—we appreciate your understanding in helping us accommodate others.

Narcotics Policy: We do not prescribe narcotics for chronic use. If ongoing pain management is needed, we'll gladly refer you to a specialized pain management practice.

Prescription Refills: The ideal time to request refills is during your evaluation appointment for personalized guidance.

Medication Changes: Any adjustments to your medications require a scheduled visit with your doctor to ensure safety and efficacy.

Refill Approvals: Most refills need your doctor's review and approval, which may take until the next weekday. Plan ahead by requesting refills well before your supply runs low.

Pharmacy Tips: Always call your pharmacy first to confirm your refill is ready—this saves you time and avoids unnecessary waits.

Monitoring Side Effects: Certain medications require ongoing monitoring for potential side effects. Please honor your follow-up appointments to stay on track with your health.

Refill Requests: Submit requests during regular office hours (Monday–Friday, 9 AM–5 PM). After hours, we won't have access to your chart and may not be able to assist promptly.
