Our Mission
Our goal at Advanced Health NY & Carmel Medical Care, PC is simple: extraordinary service quality, customer service and patient care. We accomplish this by observing a common set of values and by partnering with organizations that have the finest reputation for quality. There are no shortcuts; we believe that our goals are accomplished only with a real commitment from every employee.
Our History
Founded in 1999 in Carmel NY, our office was formed to provide top quality physical medicine to the general public. We started in a small 600 square foot location on a part time bases, which we out grew in less than 1 year and then moved into a larger 1000 square foot facility which we also out grew within a year. In 2010 we moved into our present location which is a 5000 square foot state of the art facility. Between 2007 and 2010 we added several additional locations. In 2016, we decided to focus and consolidate all of our efforts renovating, expanding, and adding new services to the Carmel location.

If you are having an emergency and can not reach us, please go to the local Emergency Room or Urgent Care.

We accept most major insurances for many of our services.

Payments for co-pays, co-insurances and uncovered services are expected at the time of service.

Returned checks may be subject to a $30 fee.

Please cancel appointments with 24 hour notice to avoid a $20 cancellation fee.

We do not prescribe narcotics for chronic use. If you require ongoing use of narcotics, we will refer you to a dedicated pain management office.

The best time to get a prescription refill is at your evaluation appointment.

If you need to change your medications, please schedule a visit with the doctor.

Most refills require the doctor’s approval. It may be the next day weekday before it can be authorized. So please request a refill before your current prescription runs out.

Call your pharmacy first to see if it is ready before going there and waiting.

Some medications have potential side effects that must be monitored. Be sure to keep follow-up appointments.

Please contact us during office hours for your refill request. We will not have access to your chart after hours and we may unable to help you.
